Job summary

Northwestern Medicine is seeking a Registered Nurse Home Health for a part-time registry position in Lemont, IL. This role provides direct patient care in home health settings, with day shifts and flexible scheduling within the registry pool.

You will assess patients, develop plans of care, coordinate with multi-disciplinary teams, and educate patients and families. A BSN and IL RN license, BLS, and driving eligibility are required.

Qualifications

• BSN required and must maintain current Illinois RN licensure.
• American Heart Association BLS certification.
• Valid driver’s license with auto insurance.

Responsibilities

• Assess and records patient status and identifies patient care needs upon admission/point of care entry.
• Organizes the assessment health data so that it is accurate, complete and accessible.
• Coordinates interdisciplinary care.
• Performs joint home visits with Charge Nurse.
• Documents diagnoses and plan of care.
