Job summary

Nightingale Home Healthcare of Illinois Inc in Willowbrook, IL is seeking an experienced Home Health Registered Nurse for a part-time, per-visit role. You will provide direct patient care and case management to an assigned caseload in patients' homes according to physician orders.

Responsibilities include obtaining medical history, performing brief exams, developing and implementing care plans, coordinating with LPNs and aides, and ensuring HIPAA compliance.

Qualifications

• Licensed RN with current license.
• OASIS and WellSky (Kinser) experience preferred, training provided.
• Med-surge and wound care experience preferred.
• TB results within last 12 months and CPR certification current.

Responsibilities

• Provide direct patient care and case management in the home setting per physician orders.
• Admit patients for service and develop a plan of care.
• Obtain medical history and perform a brief physical examination.
• Coordinate care with LPNs, Home Health Aides, and Personal Care Attendants.
• Submit patient notes and visit records in a timely manner.
• Maintain HIPAA confidentiality and accurate records.
