Personal Touch Home Care in Jefferson County, IN, seeks a Registered Nurse for home care. Positions are available part-time or full-time, serving Madison and Hanover and surrounding areas with cases based in Jefferson County.

The role covers assessments, wound care, medication administration, patient education, and coordination with a care team to ensure quality, individualized care in the patient’s home.

Qualifications

• Graduate of an approved school of nursing or Bachelor’s degree of Nursing.
• Currently licensed, in good standing, to practice in Kentucky as a Registered Nurse.
• Minimum 1 year community health nursing experience or equivalent.
• Knowledge of home care nursing practices and guidelines including OASIS.
• Complies with accepted professional standards and practice.

Responsibilities

• Assessment and care planning: perform assessments and develop care plans.
• Medication administration as prescribed; monitor for adverse effects.
• Wound care management including dressing changes and infection control.
• Educate patients and families on conditions, medications and self-care.
• Monitor progress and adjust care plans based on outcomes.
• Coordinate care with physicians, therapists, social workers, and aides.
• Maintain accurate medical records and nursing documentation.
• Provide emotional support and advocacy for patients and families.
