Job summary

Beacon Home Health in Memphis, TN seeks a Registered Nurse for per diem home health care. The RN will provide and direct nursing care in patients' homes, coordinate plans of care, and ensure compliance with state laws and payer guidelines.

Flexibility to travel across Memphis and surrounding communities is required. The RN will evaluate, document, and communicate patient status, collaborate with physicians and families, and travel a practical route to multiple residences daily.

Qualifications

One year of clinical RN experience required.
Current RN license in the state where services are rendered (TN).
Current CPR certification required.
Driver's license and access to a dependable vehicle or public transportation.

Responsibilities

Create, implement, execute, and coordinate the plan of care to promote wellness and prevent re-hospitalization.
Maintain continuity of patient care by exchanging information with physician, patient, family, and resources.
Perform initial and comprehensive nursing evaluations and develop the plan of care within state guidelines.
Maintain and submit accurate documentation and clinical notes including patient condition and response to care.
Establish and maintain an efficient visit route for home visits.
Provide high quality clinical nursing services within the scope of practice from admit through discharge.
