Job summary

Amedisys is seeking a registered nurse to coordinate and oversee home health care across patient care teams. You will perform head-to-toe assessments, monitor vital signs, manage medications, and educate families on care needs.

The role requires at least one year of RN clinical experience, current state RN license, CPR certification, and a valid driver’s license. The position offers comprehensive benefits and growth opportunities.

Qualifications

• One year of clinical experience as a registered nurse (RN).
• Current RN license, specific to the state(s) you are assigned to work.
• Minimum six months home health experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Able to assess patient status and identify requirements relative to age specific needs.
• Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Gives injections, IV therapy or tube feedings.
• Administers and maintains catheters.
• Completes documentation timely and accurately.
• Supervises LPNs and HHAs.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Other duties as assigned.
