Stonerise Home Health and Hospice is seeking a Home Health RN Field Nurse in Morgantown, WV to deliver patient-centered care in home settings. You will assess needs, plan and direct services, and coordinate with an interprofessional team to ensure safety and comfort for patients and families.

Competenze

• Graduate of an accredited school of nursing.
• Active RN licensure required.
• One to two years of recent acute care experience in an institutional setting preferred.
• Bachelor’s degree with one year of home health care experience preferred.
• Must be a licensed driver with a reliable vehicle.

• Supervise LPNs and Home Health Aides in implementing the care plan.
• Develop and adjust patient education plans as needed.
• Create and modify the Home Health Aide care plan and supervise via feedback.

• Identify nursing diagnoses using health assessment data and plan care accordingly.
• Administer prescribed medications and treatments.

You will build from resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities.Some of your primary responsibilities will include:Completing an initial assessment of patient and family to determine home care needs.
