Job summary

Stonerise Home Health Morgantown is seeking a dedicated Part-time PRN LPN Field Nurse to support home health visits in Morgantown, WV. You will follow care plans, deliver treatments, and collaborate with the care team to achieve positive patient outcomes.

Requirements include an active WV and OH LPN license, Graduation from an accredited LPN program, CPR certification, and 2 years of home health or medical/surgical experience preferred.

Qualifications

• Active LPN license in Ohio and West Virginia providing patient care.
• Graduate of an accredited practical nurse or vocational nursing program.
• Maintain current CPR certification.
• Two years of nursing experience in community/home health or medical/surgical care preferred.

Responsibilities

• Participate in coordination of home health services and report identified needs to RN/Clinical Manager.
• Assess patient and caregiver education and information related to diagnosis and plan of care.
• Provide direct patient care and implement plan of care initiated by the RN.
• Document accurately and timely; use equipment efficiently.
• Contribute to professional growth and development.
