Job summary

Stonerise Home Health and Hospice is seeking an experienced RN Field Nurse to deliver high-quality in-home care. You will plan, organize, and direct home care services while focusing on patient independence and family education.

The role requires active RN licensure, 1–2 years of acute care experience, and a valid driver’s license with insured vehicle. You will collaborate with LPNs, home health aides, and other professionals to coordinate patient care in homes and communities.

Qualifications

• Graduate of an accredited school of nursing with active RN licensure.
• Bachelor’s degree (BSN) and 1 year of home health care experience preferred.
• 1–2 years recent acute care experience in an institutional setting is desirable.

Responsibilities

• Perform initial patient and family assessments to identify home care needs.
• Conduct complete physical assessments and document health history.
• Supervise LPN and Home Health Aide in implementing the care plan.
• Develop and adjust patient education plans; direct LPN to execute plans.
• Create and modify Home Health Aide care plans; supervise via feedback.
• Assess and revise care plans as patient status changes; set goals.
• Administer medications and treatments as prescribed by a physician.
• Identify discharge planning needs and coordinate before discharge.
• Serve as Case Manager when assigned by Clinical Supervisor; coordinate care.
• Prepare clinical notes and communicate patient needs with physicians.

You will build from resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities.Some of your primary responsibilities will include:Completing an initial assessment of patient and family to determine home care needs.
