Job summary

The RN Field Staff at Ohio Living is responsible for planning, coordination and delivery of quality home health and hospice care, ensuring compliance with company standards and federal, state, and local guidelines.

Requires an Ohio RN license, 1 year of acute care or medical-surgical experience, proficiency with Windows/Microsoft Office and EMR, valid driver’s license and the ability to work weekends/evenings/holidays with 24/7 on‑call availability.

Qualifications

• Current unencumbered license to practice as an RN in Ohio.
• 1 year experience in acute care or medical-surgical nursing required.
• Experience with Medicare-certified home health is a plus.

Responsibilities

• Provides ongoing assessment and nursing care to patients in homes and health care facilities.
• Develops and implements a comprehensive care plan addressing patient needs.
• Oversees and evaluates the patient’s plan of care for effectiveness.
• Ensures compliance with federal, state, and local guidelines and agency policies.
