Formación

• Active Idaho RN License required.
• BLS Certification required.
• Home Health RN experience required.
• OASIS documentation experience required.
• Homecare Homebase (HCHB) experience required.
• Valid driver’s license and reliable transportation required.

Responsabilidades

• Provide skilled nursing care in patient homes.
• Complete OASIS assessments and documentation.
• Develop and implement patient care plans.
• Coordinate care with physicians and care teams.
• Educate patients and families on care plans.
• Manage caseload and meet productivity requirements.
• Provide hospice and home health support services.
