Job summary

Sarah Bush Lincoln is seeking a Registered Nurse for Home Care to provide nursing care in accordance with IL licensure and agency policies. The role includes delivering patient-centered care, coordinating between disciplines, and ensuring safety and infection control standards.

The position requires an active RN license in Illinois, ADN, and BLS/CPR certification with the opportunity to participate in ongoing education. Schedule includes 8am-4:30pm with weekend/holiday rotation.

Qualifications

• ADN is required.
• BLS or CPR certification within 90 days of hire.
• Driver's License – State of Illinois.
• RN-Registered Nurse – Illinois Department of Financial and Professional Regulation.

Responsibilities

• Assess data and health status.Coordinate care with disciplines for patients.
• Analyze data to identify diagnoses and issues.
• Evaluate progress toward goals and report to physicians.
• Implement care and supervise care staff per plan.
• Pursue knowledge through orientation and continuing education.
• Perform therapeutic procedures as ordered by physician.
• Develop and adjust plans to meet outcomes.
• Maintain documentation of assessments and progress.
