Sentara Home Care is seeking a qualified RN Home Health Coordinator to join the Harrisonburg team. This full-time, day-shift role involves coordinating and supervising nursing care, participating in interdisciplinary team meetings, and conducting initial and periodic assessments for participants in the program.

The ideal candidate holds an Associate's Degree in Nursing, an active VA RN license, and BLS certification within 90 days, with at least 1 year of nursing experience; home health

Qualifications

• Associate's Degree in Nursing is required.
• RN license active in VA is required; BLS within 90 days is required.
• 1 year minimum nursing experience required; Home Health experience preferred.

Responsabilités

• Coordinate and supervise all nursing care for participants per program standards.
• Interact with other facility staff and participate in interdisciplinary team meetings.
• Conduct initial and periodic assessments and administer nursing care as needed.
• Advise participants on health maintenance and disease prevention; administer medications as needed.
