Qualifications

• Current and unrestricted RN license in the state of practice.
• Current BLS/CPR certification.
• Minimum one year of nursing experience preferred.
• Experience with G-tube, Trach, Ventilator, Pediatric, Homecare, Private Duty Nursing, or Critical Care patients strongly preferred.

Responsibilities

• Deliver skilled nursing care to pediatric and/or adult patients in home settings.
• Assess, plan, implement, and evaluate individualized patient care plans.
• Provide care for patients with G-tubes, tracheostomies, and ventilators.
• Administer medications and treatments as ordered by physicians.
• Monitor patient condition and communicate changes to physicians and caregivers.
• Educate patients and family on disease management, medications, and home care needs.
• Collaborate with interdisciplinary healthcare teams to ensure coordinated care.
• Maintain accurate and timely clinical documentation.
• Assist with care coordination, goal setting, and discharge planning when appropriate.
