Qualifications

Responsibilities

• Uses nursing process to plan, implement, and evaluate patient care for contracted clients.
• Initiates care plans and requests revisions as patient needs change.
• Performs therapeutic, preventive, and rehabilitative nursing actions per diagnosis and goals.
• Responds to patient condition changes and reports to physician or supervisor.
• Administers medications and treatments as prescribed and documents effectiveness.
• Reviews care plans and protocols to identify education, coordination, and discharge planning needs.
