Job summary

Virtua in New Jersey is seeking an experienced Home Health Registered Nurse to assess, plan, implement, and evaluate patient care in patients' homes. You will coordinate with physicians and families, supervise home health aides, and ensure efficient use of agency resources.

The ideal candidate has NJ RN licensure, 1–2 years of hospital or community health experience, and strong documentation and safety practices.

Qualifications

Must have a valid NJ RN License.
1-2 years recent work experience in a hospital setting (Med/Surg, ER, OB/GYN, Peds, MICU, CCU) or community health.
Broad knowledge of nursing practice and ability to apply skills in home health.

Responsibilities

Conducts comprehensive patient assessments including psychosocial and physiological status and health behaviors.
Coordinat es plan of treatment with the physician through timely communication.
Administers, monitors, and documents therapeutic interventions.
Maintains documentation accuracy and uses point-of-service devices and timelines.
Educates patients and families about disease processes, care needs, medications, safety, etc.
Collaborates with the healthcare team to establish the plan of care.
Maintains standard precautions and patient safety.
Identifies and participates in performance improvement activities.
Maintains professional competencies at the departmental level.
