Job summary

The Roads Home Health Care Inc is seeking a dedicated Home Health Registered Nurse (RN) to deliver skilled nursing care in patients' homes across South Florida. Working with a Case Manager, you will plan, coordinate, and document care for an assigned caseload while providing hands-on patient care.

You will assess conditions, manage pain, administer medications, and participate in multidisciplinary care conferences.

Qualifications

• Active RN license in Florida.
• Current BCLS/CPR certification.
• Minimum one year of acute care nursing experience.
• Prior home health experience preferred.
• Valid Florida driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes, following the individualized plan of care.
• Assess each patient's condition, updating and adjusting the plan of care as needed.
• Assess and manage patients' pain, educating patients and families.
• Administer medications correctly based on patient age and status.
• Obtain and follow physician orders, reporting changes promptly.
• Participate in case conferences to discuss patient progress and coordinate care.
• Teach, supervise, and counsel patients and families on nursing care needs.
• Complete accurate documentation and charting for every visit.
