Job summary

The Roads Home Health Care is seeking a dedicated Home Health Registered Nurse (RN) to provide skilled nursing care to patients across South Florida. Working with a Case Manager, you will plan, coordinate, and document care for an assigned caseload while delivering hands-on nursing support to meet individual needs.

Active Florida RN license, CPR/BCLS, and at least one year of acute care nursing experience are required.

Responsibilities

• Provide skilled nursing care in patients' homes.
• Assess patients' condition and update the plan of care as needed.
• Manage pain and educate patients and families on procedures.
• Administer medications as prescribed, ensuring patient safety.
• Obtain physician orders and report status changes promptly.
• Participate in case conferences to coordinate multidisciplinary care.
• Teach and supervise patients and families on nursing care needs.
• Complete timely, accurate documentation for every visit.
