Job summary

Palm Beach - The Roads Home Health is seeking a dedicated Home Health Registered Nurse (RN) to provide skilled nursing care across South Florida. You will plan, coordinate, and document care for an assigned caseload, working with a Case Manager to deliver hands-on patient care.

The role requires an active Florida RN license, BCLS/CPR certification, and at least one year of acute care nursing experience. Home health experience is preferred and a valid Florida driver's license is required.

Qualifications

Active Registered Nurse (RN) license in Florida.
Current BCLS/CPR certification.
Minimum one year of acute care nursing experience.
Prior home health experience preferred.
Valid Florida driver's license, auto insurance, reliable transportation.

Responsibilities

Provide skilled nursing care in patients' homes per individualized plan of care.
Assess conditions and update plans as needed.
Manage and educate on pain; educate patients and families.
Administer medications following best practices.
Obtain physician orders and report changes promptly.
Participate in case conferences and coordinate multidisciplinary care.
Document visits accurately and timely.
