Job summary

Altra Homecare Group in Speedway, IN is seeking a Registered Nurse (RN) to deliver skilled home health care, working with patients in their homes and following personalized care plans that align with Indiana Medicaid requirements.

You will provide wound care, medication administration, chronic disease management, and patient education while coordinating with physicians and the interdisciplinary team, documenting in the EHR, and supporting Medicaid audits.

Qualifications

• Must hold an Indiana RN license.
• At least 1 year of nursing experience; home health and Medicaid experience preferred.
• Solid understanding of Indiana Medicaid policies and documentation standards.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Strong communication and organizational skills; ability to document accurately.

Responsibilities

• Provide skilled nursing services including wound care, medication administration, chronic disease management, and patient education.
• Ensure services are medically necessary and properly documented to meet Medicaid reimbursement standards.
• Coordinate care with physicians, case managers, and other team members.
• Empower patients and caregivers with knowledge on disease processes, medications, and self-care strategies.
• Maintain accurate, timely, and compliant documentation in the EHR.
• Participate in Medicaid audits, quality assurance initiatives, and case conferences.
• Connect patients with community resources and support services.
