Job summary

Altra Homecare Group is seeking a dedicated Registered Nurse (RN) to deliver skilled home health services in Indiana. The role focuses on patient-centered care in patients' homes while ensuring compliance with Medicaid requirements.

You will collaborate with physicians, case managers, and the care team to implement personalized care plans, perform wound care, med administration, and patient education, and maintain compliant EHR documentation. Travel to patient homes is required.

Qualifications

• Current and unrestricted RN license in Indiana.
• At least 1 year of nursing experience; home health and Medicaid experience preferred.
• CPR certification required.
• Valid driver's license and reliable transportation.
• Strong communication and organizational skills.
• Familiarity with electronic documentation systems preferred.

Responsibilities

• Provide skilled nursing services, including wound care, medication administration, chronic disease management, and patient education.
• Ensure all services are medically necessary and properly documented to meet Medicaid reimbursement standards.
• Coordinate care with physicians, case managers, and other members of the interdisciplinary team.
• Empower patients and caregivers with knowledge on disease processes, medication regimens, and self-care strategies.
• Maintain accurate, timely documentation within the electronic health record (EHR).
• Take part in Medicaid audits, quality assurance initiatives, and case conferences.
• Champion patient needs and help connect them with community resources and support services.
