Job summary

Altra Homecare Group seeks a dedicated Registered Nurse (RN) to deliver skilled home health services in patient homes, while ensuring Indiana Medicaid compliance. You will partner with patients and care teams to implement personalized care plans that promote health, independence and quality of life.

The role requires 1+ year nursing experience (home health/Medicaid preferred), CPR certification, valid driver's license, and strong communication skills.

Qualifications

• Current and unrestricted RN license in Indiana or state where employed.
• 1+ year of nursing experience; home health and Medicaid experience preferred.
• Familiarity with Indiana Medicaid policies and documentation standards.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong communication and organizational skills.
• Experience with electronic documentation systems preferred.

Responsibilities

• Provide skilled nursing services including wound care, med administration, chronic disease management, and patient education.
• Ensure services are medically necessary and properly documented to meet Medicaid reimbursement standards.
• Coordinate care with physicians, case managers, and the interdisciplinary team.
• Educate patients and caregivers on disease processes, meds, and self-care.
• Maintain accurate, timely, and compliant documentation in the EHR.
• Participate in Medicaid audits, QA initiatives, and case conferences.
• Connect patients with community resources and support services.
