Job summary

Heartland Home Health, a Gentiva company, seeks a dedicated Registered Nurse to provide skilled home health care in Midland and Bay City Counties. You will serve as the primary nurse for assigned patients, managing their individualized Plan of Care and ensuring safety and independence at home.

Qualifications

• Active RN license in the state of employment or eligibility to obtain.
• 1+ year of high acuity nursing experience.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as the primary nurse for assigned patients and manage individualized plans of care.
• Provide and document skilled nursing care including medications, wound care, and education.
• Complete assessments and reassessments to monitor progress and adjust care plans.
• Coordinate care with physicians, families, and home health team.
• Educate patients and families on health management and safety in the home.
• Communicate updates to the care team to ensure timely interventions.
• Maintain compliant clinical documentation and support quality initiatives.
