Qualifications

• Graduate of an accredited School of Nursing.
• Bachelor of Science in Nursing preferred.
• 1–2 years of medical/surgical nursing experience preferred.
• Write, speak, understand English.
• Basic nursing competencies.

Responsibilities

• Schedule completes weekly schedule and makes scheduled home visits per assignment. Schedules and prioritizes nursing visits according to client acuity and in accordance with the organization's emergency preparedness plan.
• Assessment/Documentation Admission Visit: For each admission to caseload, develops an individualized plan of care in collaboration with patient/family/caregiver; performs admission physical and psychosocial assessment to determine appropriate plan of care.
• Reassessment/Visit Documentation: For each visit, demonstrates skills to implement the plan of care, nursing interventions, and procedures; updates the plan of care as goals are met.
• Discharge/Documentation Discharge: Discharge documentation shows improvement or variances; patient prepared for discharge; inter-disciplinary agreement on plan.
• Medication: Knowledge of policies for administering and documenting medications; reconciliation each visit; review and update medication profile; DRR run; teach medication management to patient/family.
• Case Management: Coordinates plan of care for each patient; involve family; communicates with disciplines to ensure quality of care.
• Safety Awareness: Fosters culture of safety; adheres to infection control guidelines.
• Professional Development: Participates in opportunities to maintain and enhance nursing image.
• Leadership Development: Serves in leadership role.
• Advanced Clinical Skills: Applies evidence-based practice; ongoing professional development.
