Job summary

South Shore Health is seeking a community health nurse in Rockland, MA. The role provides nursing care in the community, with daily visits, discharge planning, and ongoing assessment. Licensed in Massachusetts, BSN preferred, 1–2 years med/surg experience recommended.

Mon-Fri 8:30–4:30, plus one weekend every four weeks and one holiday per year. Responsible for documentation, medication reconciliation, and collaboration with families and care teams to ensure safe, effective care in the home

Qualifications

• Graduate of an accredited School of Nursing.
• Bachelor of Science in Nursing preferred.
• 1–2 years of medical/surgical nursing experience preferred.
• Write, speak, understand English.

Responsibilities

• Schedule completes weekly schedule and home visits per assignment.
• Develops an individualized plan of care for admissions with patient/family/caregiver.
• Perform admission physical and psychosocial assessment to determine care plan.
• Document reassessments and updates to the plan of care.
• Manage medications and reconcile at each visit with DRR check.
• Coordinate case management and communicate with disciplines for quality care.
• Foster a culture of safety and adhere to infection control guidelines.
• Engage in professional development to maintain and advance nursing practice.
• Demonstrate leadership in formal or informal roles.
• Apply advanced clinical skills and evidence-based practice.
