Job summary

VitalCaring Group is seeking an RN to lead in-home patient care across a field-based territory. You will perform comprehensive assessments, develop care plans, and deliver skilled nursing services in patients' homes to support recovery and independence.

The role emphasizes clinical autonomy, collaboration with a multidisciplinary team, and a focus on reducing hospitalizations while maintaining high-quality documentation.

Qualifications

• Active RN license (or compact eligibility) + CPR certification
• Graduate of an accredited nursing program
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred)

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans in collaboration with physicians and care teams
• Deliver skilled nursing care, including medication management, treatments, and interventions
• Serve as the case manager, coordinating care across therapists, aides, and physicians
• Monitor patient progress and make clinical decisions based on changing conditions
• Educate patients and families to support disease management and independence
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely, accurate documentation in the EMR
• Supervise and support Home Health Aides as needed
• Participate in on-call rotation as appropriate
