Orion Home Health in Morton Grove, IL is seeking a Home Health RN Clinical Supervisor to lead our patient-centered team. You will oversee home health nurses, coordinate care, and ensure regulatory compliance while driving quality outcomes.

The ideal candidate is an Illinois-licensed RN with a BSN, 3+ years of recent RN experience (2+ years in home health), and strong leadership, communication, and critical thinking skills. Travel to patient homes is required.

Responsabilidades

• Supervise and coordinate the activities of a team of home health nurses, therapists, and other healthcare professionals to ensure effective and efficient patient care delivery.
• Conduct patient assessments, develop and implement care plans, and provide guidance and support to staff members.
• Investigate and resolve patient complaints, ensure compliance with regulatory requirements, and maintain accurate records and reports.
• Collaborate with physicians, social workers, and other healthcare professionals to ensure seamless communication and coordination of patient care.
• Participate in on-call rotations, respond to emergencies, and provide guidance to staff members after hours.
• Develop and implement quality improvement initiatives, monitor patient outcomes, and identify areas for improvement.
• Recruit, train, and mentor staff members to promote professional growth and development.

Requirements

• Excellent clinical knowledge and critical thinking skills, with the ability to analyze complex patient situations and develop effective care plans
• Ability to lift up to 25 pounds, travel to patient homes, and work in a dynamic environment
