Qualifications

• Associate or bachelor's degree in nursing.
• Current RN license for the assigned state.
• 1+ year of clinical RN experience in home health or hospice.
• 1+ year of RN management experience.
• Current CPR certification; driver's license required.

Responsibilities

• Recommends employment decisions for staff to the director of operations.
• Oversees quality of patient care and clinical performance of nurses and aides.
• Organizes clinical operations for the care center.
• Reviews service requests and coordinates referrals/admissions with intake.
• Oversees scheduling and payor verification requirements.
• Ensures timely and complete documentation across staff.
• Assists in establishing Plan of Care goals with clinicians.
• Monitors patient needs and revises the care plan as needed.
• May provide direct patient care during emergencies.
• Participates in the on-call process and staff training.
