Stonerise Home Health and Hospice in Charleston, WV is seeking a Home Health RN Clinical Manager to lead a multidisciplinary team and ensure high-quality patient care at home. You'll oversee care plans, coordinate between physicians and caregivers, and drive compliance with Medicare guidelines.

The role emphasizes clinical oversight, staff recruitment and training, and ongoing performance improvement to maintain exceptional patient outcomes.

Qualificações

• Active RN license in the state of West Virginia.
• Minimum of three (3) years of home care experience.
• Minimum of one (1) year of experience in clinical management.

Responsabilidades

• Audit charts for clinical oversight and Medicare compliance.
• Manage clinical activities including assessments and care plans.
• Supervise clinical team and participate in recruitment and orientation.
• Ensure service quality and care coordination among caregivers and physicians.
