Job summary

Phoenix Home Care and Hospice in the St. Louis area is seeking a Home Health RN Case Manager. The role offers a base of $78,000-$88,000 plus a $10,000 sign-on and a pay-per-visit structure.

Schedule is Monday-Friday with limited on-call, and you will manage a caseload while guiding LPNs and coordinating with therapy teams. The position provides autonomy to set your own visit schedule, AI-assisted charting via Roger to reduce time, and paid training for 4-6 weeks.

Qualifications

• Missouri or Compact RN license required.
• Valid driver's license required.
• Strong organizational, clinical, and communication skills.
• Ability to manage a caseload and coordinate care across therapies.

Responsibilities

• Manage a caseload of home health patients.
• Supervise LPN visits and coordinate with PT/OT/Speech teams.
• Conduct assessments and build personalized care plans.
• Support patients with CHF, COPD, diabetes, wounds and medications.
