Presbyterian Homes & Services – Optage Home Health is seeking an RN Case Manager for Admissions and Start of Care in the Twin City metro area. The role is full-time with daytime hours and an equitable on-call rotation, with a compassionate Christian environment and strong leadership support.

Ideal candidates are licensed RNs, preferably with a BSN, and have 3+ years of nursing experience including hospice/home health; strong communication and teamwork skills are essential.

Qualifications

• Current registered nurse licensure in state practiced.

Responsibilities

• Deliver quality and safe home health services in patient homes, SNF, or assisted living.
• Develop, implement and evaluate patient care plans; revise as needed.
• Coordinate hospice services and ensure continuity of care with other professionals.
• Assess physical, psychological, social, and spiritual needs of patient and family.
• Educate, supervise and counsel patient and caregiver about care needs.
• Document patient/family services accurately and promptly.

We are seeking a full-time Registered Nurse to join our team as an RN Case Managerwho willprimarily focus on completing Admissions and Starts of Care (SOC)in the Twin City metro area.In this position you will play a critical role in caring for older adults clinical needs - not only physically, but emotionally, socially, mentally, and spiritually. This is a full-time day position Monday through Friday, as well as participation in an equitable on call rotation.

Optage Home Care is a well-established program that has been serving older adults and their families for over 15 years. As a part ofPresbyterian Homes & Services, we are a mission driven, faith-based organization.

Responsibilities

The RN Case Manager ensures quality and safe delivery of home health services to our clients in their home, skilled nursing facility or assisted living setting. Our Case Managers work to develop, implement and evaluate the plan of care and make necessary revisions to address all problems. They provide coordination of hospice services, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs. Case Managers utilize assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life for the patient/family.

Essential tasks and duties include providing ongoing assessments of physical, psychological, social, and spiritual needs of the patient and family, provide education, supervision and counseling to the client and their family/caregiver regarding care needs as appropriate to client’s needs, providing accurate and appropriate documentation of patient/family services.

Qualifications

• Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
• BSN preferred.
• A minimum ofthree years of nursing experience preferred.
• Critical care/oncology, long-term care, or medical/surgical background with strong knowledge of pain and symptom management preferred.
• Hospice/Home Care experience preferred.
• Excellent interpersonal skills.
• Start of Care (Oasis) Experience Preferred
• Ability to work as a member of an Interdisciplinary Team.
• Experience with symptom management.
• Strong oral and written communications skills.
• Computer skills helpful.
• Reliable transportation, current driver’s license and current auto insurance.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Pay Range

The estimated starting pay range for this role is $82,813 - $108,000 per year. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.
