Job summary

Presbyterian Homes & Services—Optage Home Health seeks a full-time RN Case Manager to complete Admissions and Starts of Care in the Twin Cities metro area. This day role includes an on-call rotation and focuses on holistic care for older adults in home settings.

Qualifications include active RN licensure in the state, BSN preferred, and 3+ years of nursing experience, with hospice/home care experience advantageous. Strong communication and teamwork are essential.

  • Assess physical, psychological, social, and spiritual needs and educate, supervise and counsel clients and families.
  • Document patient/family services accurately and timely.

Presbyterian Homes & Services – Optage Home Health is looking for registered nurses who feel called to serve older adults within a caring Christian environment. We are seeking a full-time Registered Nurse to join our team as an RN Case Manager who will primarily focus on completing Admissions and Starts of Care (SOC) in the Twin City metro area. In this position you will play a critical role in caring for older adults clinical needs - not only physically, but emotionally, socially, mentally, and spiritually. This is a full-time day position Monday through Friday, as well as participation in an equitable on call rotation.

Optage Home Care is a well-established program that has been serving older adults and their families for over 15 years. As a part of Presbyterian Homes & Services , w e are a mission driven, faith-based organization.
Responsibilities

The RN Case Manager ensures quality and safe delivery of home health services to our clients in their home, skilled nursing facility or assisted living setting.
Our Case Managers work to develop, implement and evaluate the plan of care and make necessary revisions to address all problems. They provide coordination of hospice services, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs. Case Managers utilize assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life for the patient/family.
Essential tasks and duties include providing ongoing assessments of physical, psychological, social, and spiritual needs of the patient and family, provide education, supervision and counseling to the client and their family/caregiver regarding care needs as appropriate to client’s needs, providing accurate and appropriate documentation of patient/family services.

Qualifications
  • Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • BSN preferred.
  • A minimum of three years of nursing experience preferred.
  • Critical care/oncology, long-term care, or medical/surgical background with strong knowledge of pain and symptom management preferred.
  • Hospice/Home Care experience preferred.
  • Excellent interpersonal skills.
  • Start of Care (Oasis) Experience Preferred
  • Ability to work as a member of an Interdisciplinary Team.
  • Experience with symptom management.
  • Strong oral and written communications skills.
  • Computer skills helpful.
  • Reliable transportation, current driver’s license and current auto insurance.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
Pay Range

The estimated starting pay range for this role is $82,813 - $108,000 per year. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.

Full-time benefits-eligible employment begins at a minimum of 30 hours per week:
