Job summary

Qualifications

• Registered Nurse (RN) with active license and board of nursing clearance.
• 1-3 years in home health or hospice preferred.

Responsibilities

• Functions as nursing lead in home care, assessing patient needs.
• Implements nursing process during Start of Care visits and re-evaluations.
• Collaborates with physicians and interdisciplinary team to plan care.
• Makes recommendations to optimize resource use and patient outcomes.
• Initiates changes in care plans and documents Significant Change in Condition.
• Educates patients and caregivers about disease management and self-care.
