Amedisys, Inc. is seeking a Registered Nurse Case Manager to join our team delivering home health services in Massachusetts. This role requires clinical RN experience, an active RN license for the state, CPR certification, and a valid driver's license.

Qualifications

1+ year of clinical RN experience
Current RN license for the assigned state(s)
Current CPR certification
Valid driver’s license and reliable transportation

Responsibilities

Performs patient assessments and coordinates with the care team to develop and implement a plan of care.
Makes referrals to other disciplines as indicated by patient needs or documents rationale.
Educates patients to promote health and independence and teaches self-care techniques.
Supervises LPNs and HHAs and ensures timely documentation.
Communicates patient progress to the clinical manager and care team.
Participates in staff education and clinical development.
On-call responsibilities to patients/families as assigned.
