Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one nursing care in patients' homes. You will develop personalized care plans with physicians, monitor conditions, and educate families, while documenting care and coordinating with the care team.

The role reports to the Clinical Manager and includes collaboration with physicians and clinicians to support recovery and independence for patients in their homes.

Qualifications

• RN with med-surg/ICU/ER acute care experience.
• Current, unrestricted RN licensure.
• CPR certification on file.
• Ability to work with a care team across disciplines.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes.
• Develop and implement individualized care plans with physicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatments.
• Maintain accurate patient records and coordinate with providers.
• Coordinate with the Clinical Manager and team.
