CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one nursing care in patients' homes under the Clinical Manager's supervision. You will develop and implement individualized care plans with physicians and healthcare professionals, monitor patient progress, educate families, and coordinate care with the broader team.

The role involves supervising Home Health Aides, conducting assessments, and ensuring documentation accuracy.

Competenze

RN licensure (current and unrestricted)
CPR certification (current)
Experience collaborating with a team of healthcare professionals
Valid driver's license, auto insurance and reliable transportation

Mansioni

Provide skilled nursing care to patients, working onsite in their homes, including administering medications, performing wound care, and conducting patient assessments
Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals
Monitor patients' conditions and report changes
Educate patients and their families on disease management, medication, and treatment options
Maintain accurate records of patient care and coordinate with other healthcare professionals
Report patient care and condition progress to patient's physician and Clinical Manager
Oversee the work of Home Health Aides and monitor their documentation
