Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse in Brooklyn Park, MN. You will provide one-on-one skilled nursing care in patients' homes, collaborating with physicians to develop care plans and monitor progress to support independence.

This role requires an active RN license, CPR, and Med/Surg/ICU/ER or acute care experience. You will educate families, maintain accurate records, and supervise Home Health Aides in a high-quality home-care setting.

Qualifications

Med surg, ICU, ER, or acute care experience.
Current and unrestricted Registered Nurse licensure.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Valid driver’s license and reliable transportation.
One year nursing experience (preferred).
Home Health experience (preferred).

Responsibilities

Provide skilled nursing care to patients in their homes, including medications, wound care, and assessments.
Develop and implement individualized care plans with physicians and other healthcare professionals.
Monitor patients’ conditions and report changes.
Educate patients and families on disease management, medication, and treatment options.
Maintain records and coordinate with other healthcare professionals.
Report patient progress to physician and Clinical Manager.
Oversee Home Health Aides and ensure documentation completion.
