Caretruly Healthcare LLC. is seeking a Case Manager (RN) to provide skilled nursing care to patients in their homes in accordance with agency policies and state and federal regulations. You will implement individualized care plans, supervise Personal Care Workers (PCWs), coordinate services, and support patients and families to achieve optimal health outcomes.

Qualifications

• Active RN license in Wisconsin.
• Minimum of 1 year clinical experience; home health preferred.
• Proficiency in EMR documentation and charting.

Responsibilities

• Provide skilled nursing care in patients' homes per policy and regulations.
• Develop, implement and update individualized care plans with the team.
• Supervise Personal Care Workers (PCWs) and ensure care plan adherence.
