Job summary

Mid Michigan Home Health & Hospice is seeking a compassionate Home Care & Hospice Registered Nurse (RN) to serve Lansing, MI and surrounding communities. You will provide in-home skilled nursing, develop and evaluate care plans, manage medications, and coordinate care with a multidisciplinary team to support patients and families.

The role offers flexible scheduling in a home-based setting, with opportunities to grow in home health and hospice.

Qualifications

• Current, unrestricted Michigan RN license.
• Home health, hospice, or acute care experience preferred.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and organizational skills.
• Ability to work independently with a flexible daily schedule.
• Valid driver's license, reliable transportation, and automobile insurance.
• Compassionate, patient- and family-centered care approach.

Responsibilities

• Provide skilled nursing assessments and direct home-based patient care.
• Develop, implement, and evaluate plans of care.
• Perform comprehensive assessments and monitor condition changes.
• Manage medications, wound care, and disease education.
• Coordinate care with physicians, therapists, social workers, and team.
• Deliver compassionate hospice care and end-of-life support.
• Educate patients and caregivers on disease processes and safety.
• Complete accurate clinical documentation per policies and regulations.
• Communicate effectively with patients, families, physicians, and management.
