Job summary

The RN Case Manager will supervise staff, ensure quality patient care, and collaborate with clinicians to deliver the individual plan of care. The role requires an RN licensure in Texas, CPR certification and at least one year of RN experience.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as a registered nurse in TX.
• Minimum of one year of experience as an RN.
• Current CPR Certification required.

Responsibilities

• Assess nursing needs and manage the patient care plan.
• Complete initial patient assessment upon admission, including history.
• Selects appropriate nursing diagnoses and coordinates interventions.
• Evaluate medicines for adverse effects and allergies.
• Set realistic, measurable goals aligned with diagnosis.
• Inform physician of assessments and recommended interventions.
• Document progress and discharge notes timely.
• Notify physician of changes in patient condition and needs.
