AccentCare, Inc. is seeking an RN Case Manager for home health coverage in Downtown and North Jacksonville. The role combines direct and indirect patient care to manage care plans from admission through discharge.

The ideal candidate is an RN licensed in the state, with CPR, at least one year of RN experience, and the ability to coordinate interdisciplinary teams while meeting patient goals. Pay ranges from $76,000 to $85,000 annually, with a Mon-Fri schedule and on-call duties.

Qualifications

• Licensed RN in the state of operation.
• Minimum of one year RN experience.
• Current CPR Certification required.

Responsibilities

• Assess nursing needs and manage the patient’s care plan.
• Complete initial patient assessment at admission, including history.
• Select appropriate nursing diagnoses.
• Assess medications for interactions, allergies, and contraindications.
• Set realistic, measurable goals for diagnosis and plan.
• Inform physician of assessment results and recommended interventions.
• Implement nursing interventions per diagnosis and goals.
• Re-evaluate patient needs as required.
• Document assessments, progress, and discharge notes in a timely manner.
• Inform physician and staff of changes in patient condition.
