Amedisys, part of the Optum family, is hiring a Nurse Case Manager to guide patients and families through the care continuum. Under supervision, you will assess needs, coordinate care, support discharge planning, and manage outpatient care with education and support for patients and caregivers.

Responsibilities include conducting assessments, coordinating with the care team, supervising staff, maintaining documentation, and participating in ongoing education to ensure high-quality patient

Formación

• Current RN license (state-specific)
• Current CPR certification
• 1+ year of clinical RN experience
• Valid driver’s license with reliable transportation

Responsabilidades

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care
• Make referrals to other disciplines as indicated by patient needs
• Promote patient health and independence through teaching and rehabilitation
• Supervise LPNs and HHAs
• Complete documentation timely and accurately
• Regularly communicate patient progress to the clinical manager and team
• Plan and provide staff education
• Perform on-call responsibilities as assigned
• Participate in clinical development and continuing education programs
• Other duties as assigned
