Amedisys, part of the Optum family, is hiring an RN for Home Health. You will guide patients and families through care planning, coordinate services, and support transitions in a home-based setting.

The role offers a total cash target pay range of $79,222 to $118,833 annually, plus per-visit and hourly pay components. On-call duties and comprehensive benefits are included where eligible.

Qualifications

• Current RN license, state(s) permitted
• CPR certification
• 1+ year of clinical RN experience
• Valid driver's license and reliable transportation

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by patient needs
• Promotes patient health and independence through teaching and rehabilitation
• Supervises LPNs and HHAs
• Completes documentation timely
• Communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Participates in on-call responsibilities and education programs
• Other duties as assigned
