Job summary

Angels Care Home Health in Amarillo, TX is seeking an experienced Registered Nurse (RN) Case Manager to coordinate all aspects of patients' home care. You will interact with other medical disciplines as needed and provide professional care to the patient in the home setting.

You will also perform the admission visit to determine eligibility for home care services and develop the plan of care to be followed, ensuring quality and compliant care across the patient’s continuum.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state Registered Nurse license.
• Valid driver’s license and reliable transportation.
• One year RN experience preferred; CPR and hepatitis consent/declination.
• Proof of auto liability insurance and driving eligibility.

Responsibilities

• Coordinate all aspects of patient home care and interact with medical teams.
• Perform admission visits to determine eligibility and plan of care.
• Develop and implement individualized care plans in collaboration with physicians and caregivers.
• Ensure compliance with regulatory and safety standards in home health services.
