Job summary

Amedisys, Inc. is seeking a Registered Nurse Case Manager to provide comprehensive in-home care across Hamilton County.

You will assess patients, collaborate with the care team to develop and implement care plans, supervise LPNs and HHAs, and ensure timely documentation while supporting patient education and independence in their home setting. The role emphasizes patient-centered care, coordination with clinicians, and ongoing professional development within a leading home health and hospice

Qualifications

• RN license, CPR certification, home health experience preferred.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement plan of care.
• Makes referrals to other disciplines as needed.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities as assigned.
• Participates in continuing education.
