Job summary

Amedisys is a leading home health and hospice provider in the United States seeking an RN Case Manager in New Jersey. The role is full-time days with a competitive annual salary range of $95,000 to $100,000.

Responsibilities include patient assessments, care planning, and collaboration with the care team to ensure high-quality patient care at home. Qualifications require 1 year of nursing experience, active RN license in the state of assignment, and CPR certification.

Qualifications

• One year of nursing experience required.
• Current RN license for the state of assignment.
• Current CPR certification is required.
• Valid driver’s license, reliable transportation, and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy, timeliness, and according to center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
