Mission Healthcare, the leading home health and hospice provider in the western United States, seeks a dedicated Registered Nurse for Ventura, CA. You will perform comprehensive patient assessments, initiate and adjust plans of care, and continuously re-evaluate patient needs within a home health setting.

Responsibilities include administering medications, educating patients and families, coordinating care, and planning for discharge.

Qualifikationen

• Must hold current and valid RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS) certification with CPR.
• Valid driver's license and auto insurance.
• Associate's or Bachelor’s degree in Nursing with 1 year home health experience preferred.

Aufgaben

• Complete initial patient and family assessment to determine home care needs.
• Initiate and revise the plan of care as patient status changes.
• Regularly re-evaluate patient needs and outcomes.
• Use health data to determine nursing diagnoses.
• Develop and implement a care plan with goals based on nursing diagnosis.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family on plan of care and needs.
• Provide health care instructions per assessment and plan.
• Identify discharge planning needs and begin ahead of discharge.
