Job summary

Amedisys is seeking a dedicated RN Case Manager in Texas to support home-based patients in a full-time day schedule. You will perform patient assessments, coordinate with the care team, and develop individualized care plans that help patients heal where they are most comfortable.

Qualifications

Minimum of 1 year of nursing experience.
Current RN license in applicable state(s).
Current CPR certification required.
Valid driver's license with reliable transportation and liability insurance.

Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to hospice care disciplines as needed or documents rationale for not doing so.
Develops and maintains individualized patient care progress records per center policies.
Records pain and symptom management changes and outcomes.
Teaches patients, families, and caregivers appropriate patient care techniques.
Communicates regularly with clinical manager and care team.
Performs on-call responsibilities and on-call services as assigned.
Other duties as assigned.
