Qualifications

• RN with 1+ year of clinical experience.
• Current RN license for the assigned state.
• CPR certification is required.
• Valid driver's license with reliable transportation.

Responsibilities

• Assess patients and collaborate with the care team to plan and implement care.
• Make referrals to other disciplines as needed and document rationale.
• Promote patient health through teaching and self-care training.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Regularly report patient progress to the clinical manager.
