Job summary

Amedisys is seeking a registered nurse case manager in Georgia to assess patients at home, develop and implement plans of care, and coordinate with the care team. You will support patient independence through teaching and rehabilitative measures while supervising LPNs and HHAs.

Qualifications

• 1+ year of clinical experience as a Registered Nurse (RN).
• Current RN license for the state(s) assigned to work.
• Current CPR certification.
• Valid driver's license and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient's needs.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans staff education and participates in ongoing clinical development.
• Performs on-call duties as assigned.
