Interim HealthCare of the Black Hills is seeking an RN Case Manager to join our Home Health team in Hot Springs, SD. The role offers part-time and full-time opportunities for compassionate RNs to develop personalized care plans and coordinate with an interdisciplinary team, delivering care in patients' homes.

You will perform skilled nursing visits, complete OASIS assessments, educate families, monitor progress, and advocate for patients while maintaining high standards of safety and quality.

Qualifikationen

• Licensed Registered Nurse (RN) in South Dakota.
• Home Health experience preferred but not required.

• Perform skilled nursing assessments and create individualized care plans.
• Caseload management from admission to discharge.
• Coordinate with physicians, therapists, social workers, and aides.
